This is a non-binary patient who requested a "button-hole” top surgery procedure, with the specific goal of maintaining sensation, as well as leaving extra tissue. They did not want an overly masculine chest, and wanted to maintain some degree of fullness. No skin grafts were used; instead the nipple-areolar complexes remained attached to underlying nerves and blood vessels. These were specific goals to match their non-binary gender identity and gender expression. This patient is 2 months post op—the redness of the incisions will fade by 1 year.
